Our CONNECTPlus app gives patients the information and remote monitoring tools they need to help them to manage their multiple conditions and elective care pathways so that they can look after themselves better at home, thereby reducing the demand on the health system.

Our CONNECTPlus app offers: Multiple Conditions - Support for multiple conditions in a single place. Information AND data - it provides patients with information to help them to manage their condition as well as symptom trackers to enable them to record and report on their symptom progression. Localised set up - each version of the app is set up to reflect individual hospital services and treatment pathways anywhere in the world. And it makes a difference: It has reduced calls to a nurse helpline from 70 a day to 40. 62% of new start patients for DMARDs, sub-cut Methotrexate, Biologics educated remotely through the app. A reduction in waiting times from 3 weeks to 1 week to start medication. Nurse time in medication clinics reduced from 4 hours weekly to 30 minutes. Winners of the Building Better Healthcare Award for Patient Centred Healthcare Software 2021
